As far as i remember you would always change a nappy as soon as possible if they have passed stools, as the combination of faeces and urine can lead to nappy rash.
Generally a nappy should be checked every two hours
Some change nappies before or after a feed and then there are some nurseries that agree to a minimum change policy with parents. This means that the nursery would not change the child more frequently than if she/he was at home.
