I was also confused about birth to three matters at first as I was trying to source observation sheets. The company you need to call is called Sound Learning Limited, Tel: 01543 467787. For £22 they sent me a fantastic pack of blank observation records and various planning forms etc. Along with the pack you also receive a CD Rom containing all the information.
